How do I take a Distribution or Rollover out of the plan?

Check the Plan Summary available in the Documents section to see if you are eligible for a Distribution. Download the Distribution Request Form from the Documents section, fill it out, and send it to us. Our address, fax, and email is at the top of the form.

How do I make a Hardship Withdrawal?

Check the Plan Summary available in the Documents section to see if your plan allows hardship withdrawals. Download the Hardship Withdrawal Request Form from the Documents section, fill it out, attach the required documentation, and send it to us. Our address, fax, and email is at the top of the form.

How do I take out a Loan?

Check the Plan Summary available in the Documents section to see if your plan allows loans. Download the Loan Request Form from the Documents section, fill it out, and send it to us. Our address, fax, and email is at the top of the form.

How long does it take to process?

It can take up to 15 business days upon receipt of the form in good order for the distribution to be processed. You should receive it in 2-3 business days if you elected ACH and up to 1 week or more if you elected check or rollover from the date it has been processed.

Incoming Rollovers

How do I do a rollover into the plan?

Download the Incoming Rollover Request form from the Documents section, fill it out, and send it to us. Follow the instructions on the form to tell your previous 401(k) or IRA provider who to make the check out to and where to send it.

Account Questions

Why can’t I see my account balance?

If you haven’t completed all the enrollment steps, you will not be able to see your account balance and will be prompted to finish enrolling every time you login. Once you finish enrolling, you will be able to see the dashboard with your account balance.

Why can’t I save Beneficiaries?

Please check the most common reasons why Beneficiaries might not save:

  1. Are any of the boxes orange? This indicates there is an error in something that was entered.

  2. Do the allocation percentages add up to 100%?

  3. Are all the disclaimer boxes at the bottom of the page checked? Even if your marital status is incorrect, please check the box and click save. You can then update your marital status by going to the Personal Info section.

  4. If you are adding a minor beneficiary, the Guardian’s email address cannot be the same as the Beneficiary’s email address and the Guardian’s SSN cannot contain dashes.

How do I change my password?

Go to Settings and enter your New Password. If you haven’t completed all the enrollment steps, you will not be able to see the main menu where Settings is and will be prompted to finish enrolling every time you login. Once you finish enrolling, you will be able to see the menu and click on Settings.

How do I change my email?

Your email is the email provided by your employer and is synced to payroll, which cannot be changed by you. You can create a username or add a second email address by going to Settings. What you enter in Username or Personal Email must be different from what is already in the Email box. If you haven’t completed all the enrollment steps, you will not be able to see the main menu where Settings is and will be prompted to finish enrolling every time you login. Once you finish enrolling, you will be able to see the menu and click on Settings.

How do I change my mailing address, Marital Status, or other personal info?

Go to Personal Info. If you haven’t completed all the enrollment steps, you will not be able to see the main menu where Personal Info is and will be prompted to finish enrolling every time you login. Once you finish enrolling, you will be able to see the menu and click on Personal Info.

How do I find more information about my plan?

The Plan Summary available in the Documents section will tell you everything you need to know about your plan, including eligibility requirements and distributions allowed.

How do I find more investment information about my plan?

The Investments section has Factsheets and links to the fund websites. The Fee Disclosure available in the Documents section will also give you information about the investment options available in your plan.

How do I find information about fees in my plan?

The Fee Disclosure available in the Documents section will tell you all the fees applicable to your plan.
